Insurance / Insurance Waiver

2013 - 2014 Injury - Sickness Insurance:

URI Health Services has selected Bollinger Insurance Solutions (http://www.bollingerinsurance.com/) to provide student health insurance beginning September 1, 2013 (August 15, 2013 for International Students). Monumental Life Insurance Company has underwritten the plan and claims will be administered by Bollinger Insurance Solutions.

The First Health Network www.firsthealth.com will be the preferred provider network.

The rates for the Student Accident/Injury Insurance Plan are:

  Fall 2013 Spring 2014
Student $1,636* $1,014**
Spouse $3,207* $2,034**
Child(ren) $2,421* $1,540**

*This amount covers domestic students\dependants from 9/1/2013 through 8/31/2014. International students are covered 8/15/2013 through 8/14/2014.

**This amount covers domestic students\dependants from 1/1/2012 through 8/31/2014. International students are covered 1/1/2013 through 8/14/2014.

An insurance waiver is required to be submitted annually and will be available beginning July 1, 2013.

The Spring 2013 Insurance Waiver deadline has passed. If you believe you have a reason to petition the insurance charge, please visit our insurance office in Health Services or call 401-874-4749.

You will need to provide the following information to complete the petition::

  • Name of Insurance
  • Policy Number
  • Group Number
  • Claim Address
  • Subscriber's Name and Date of Birth

2012-2013 Insurance Petition

Our 2012-2013 Insurance Waiver deadline has now passed. If you feel you have a valid reason to petition the insurance charge, please visit Health Services to speak with an administrator.

At the time of submission you will need the following:

  • Student ID Number
  • Current Health Insurance Card including Policy and Group Number
  • Subscribers Name, Date of Birth, and Relationship

(www.universityhealthplans.com) as a leading benefit brokerage/consulting firm to service the University sponsored health insurance plan beginning September 1, 2012 (August 15, 2012 for International Students). Nationwide Life Insurance Company has underwritten the plan and claims will be administered by Consolidated Health Plans (www.consolidatedhealthplan.com).

The First Health Network www.firsthealth.com will be the preferred provider network.

The rates for the Student Accident/Injury Insurance Plan are:

Spouse $3,285* $2,084**
Child(ren) $2,480* $1,578**

The school insurance currently has a pharmacy benefit. Please click on the Insurance Brochure above for details regarding this benefit.

IMPORTANT: You have 90 days to submit a valid claim form for pharmaceutical reimbursement. The pharmacy does not participate in third party payment plans.
